At Clarity Health Care, we are committed to providing safe, respectful, and person-centred mental health care. Your feedback helps us understand what we are doing well and where we can improve.
If you have a concern, complaint, suggestion, or positive experience to share, please use this form to tell us. You can provide as much or as little information as you feel comfortable sharing.
Once your submission is received, our dedicated team will review the information and respond appropriately within a reasonable timeframe. Every submission will be handled confidentially.

Complaints process
- We will acknowledge your complaint within two business days of receiving it.
- We will review your concerns thoroughly, objectively, and without bias, in line with the Mental Health Act 2014 and Charter of Human Rights and Responsibilities Act 2006.
- We will listen to your experience in a respectful and person-centred way and ensure your views and preferences are appropriately considered.
- You will not be disadvantaged or treated differently for making a complaint.
- Your complaint will be managed confidentially, and details will only be shared with your consent, or when required by law or necessary to ensure your safety.
- We aim to provide a clear response within 30 days. If more time is required due to the complexity or risk level of the matter, we will keep you updated.
- We will work toward resolving your complaint in a way that is fair, reasonable, and appropriate to the issues raised.
- Your feedback is important and will be used to improve our services, strengthen processes, and help prevent future issues.
Appeals
If you are unhappy with the outcome or how your complaint has been managed, you can request an internal review by our Clinical Director or Chief Executive Officer.
