What is social prescribing?
Social prescribing is a person-centred, community-based approach to health and wellbeing. A GP or clinician refers a patient to a link worker, who co-designs a personalised plan connecting them to non-clinical community services — social groups, arts, volunteering and nature-based activities — addressing the social determinants of health.
Who can refer?
GPs, nurses, allied health professionals, social workers and trusted community members. Many programs also accept self-referrals.
Who benefits?
People experiencing loneliness, social isolation, mild-to-moderate mental health challenges, chronic illness, housing or financial stress, or lack of community belonging.
Hobart 2025
The Safe Haven and Mental Health Integration Hub at the Peacock Centre (North Hobart) offer free walk-in support, alongside the Glenorchy Medicare Mental Health Centre (interim telehealth while it is being established).
Evidence
Australian and international research shows social prescribing improves health-related quality of life, self-reported health and psychological distress, and reduces loneliness.
Link workers
Trained connectors who assess needs, navigate services and co-design plans — typically supporting patients over 12 weeks.
Cost
Most PHN-commissioned mental health programs are fully funded — no out-of-pocket cost to patients. Medicare Mental Health Centres and Safe Havens are always free.
