Evidenced-based Wellbeing Program

The Optimal Health Program (OHP) is an evidence-based wellbeing program designed to empower patients with the tools and strategies they need to manage their mental health effectively.

Founded upon years of research, OHP builds on an individual’s strengths and promotes self-efficacy and hope. It is centred around eight evidence-based modules which can be conducted as a group or individually by trained people over a number of weeks.

OHP offers a framework that supports a conversation about wellbeing – rather than illness – and provides the opportunity to reflect, write down ideas and respond to an individual’s needs without having to give solutions.

What makes OHP different from other chronic disease management programs is its finely tuned focus on patient self-management and self-empowerment. By providing patients with the tools they need to own their own healthcare journey, patients become better equipped to remain in control of their health and are more invested in making better lifestyle choices with regards to more optimum health and wellbeing.

Select findings include:

  • Completion of the OHP has been demonstrated to reduce hospital admissions by 50% in the 12 months post completion, as well as reduce length of stay when hospitalised. 
  • Individuals with chronic kidney disease who have completed the OHP demonstrate significantly reduced symptoms of anxiety & depression compared to controls. 
  • Individuals with diabetes who have completed the OHP report significantly improved general and disease-specific self-efficacy compared to controls. 
  • Individuals with Long Covid who have completed OHP report significantly improved wellbeing post program compared to pre intervention. 

The program’s efficacy and reliability are underpinned by numerous randomised control trials and almost 30 peer-reviewed publications across some of the world’s leading academic journals.

OHP is in alignment with key elements of consumer defined recovery, in that it is self defined and self-determined, and provides the essential mechanism of how to, through self-advocacy.

At the heart of OHP sits two important instruments: ‘I Can Do Model’ and ‘Health Plans’:

  • The I Can Do Model defines actual and potential resources to manage any adversity in life and maintain desired levels of wellbeing.  
  • Health Plans ensure personal autonomy is maintained at all times, including during episodes of illness.
